CONTACT USTo promote and provide comprehensive care, we encourage Sutter Family Practice patients to maintain all medical records from previous healthcare providers, specialists and emergency room visits in our Chatsworth office. We also encourage patients to have bi-annual check-ups, and we like to see patients who are managing chronic conditions at least 4 times a year.
Through our Total Care Program, we provide the services of a family doctor as well as a healthcare partner and advocate.

If you require the attention of a specialist, Sutter Family Practice will advise and refer you, while making every effort to coordinate covered care within your insurance network.
